What Anxiety Actually Does to Your Body
People who arrive convinced that something is medically wrong with them are not wrong about the sensations. The pounding heart, the tight chest, the stomach in knots, the hands that won't quite stop trembling: all of it is real, measurable and happening in the body. What gets misread is the cause.
Here's what I often tell clients in that moment: your body isn't broken. It's doing exactly what it was built to do. It's just doing it at the wrong time, and far too often.
Anxiety isn't only a feeling in your mind. It's a full-body event — a cascade of hormones, nerve signals, and muscle responses that were designed to keep you alive. When you understand what's happening beneath the sensations, they get less frightening. And when your symptoms feel less frightening, they tend to loosen their grip. The other half of that loop, the thought that keeps escalating while the body keeps answering it, is a separate piece of work.
The Alarm System: Fight, Flight, and Freeze
Your body is protecting you, just a little too enthusiastically.
At the center of anxiety is an ancient survival circuit. When your brain perceives a threat, real or imagined, a small almond-shaped structure called the amygdala sounds the alarm before your thinking brain has time to weigh in.
This "fight or flight" response is one of the most well-mapped systems in physiology: under threat the body mobilizes in seconds (a surge of adrenaline, redirected blood flow, a readiness to act) as detailed in a 2009 overview of the neuroendocrinology of stress (stress response review, 2009). What the lab documents is exactly what your body does when a work email lands wrong. That alarm activates your sympathetic nervous system, the "gas pedal" of your body. Within seconds:
Your adrenal glands release adrenaline and, a bit later, cortisol.
Your heart beats faster and harder to pump blood to your muscles.
Your breathing quickens to load your blood with oxygen.
Blood is redirected away from your digestive system and toward your limbs.
Your pupils dilate and your senses sharpen.
This is brilliant if you're facing a genuine predator. The problem is that your amygdala can't always tell the difference between a bear and a blunt email from your boss. It fires the same way for both.
Cortisol and the Long Game
Adrenaline gives you the immediate jolt, but cortisol is the hormone that keeps the pressure on. It's meant to help you stay alert and mobilize energy during a crisis, then taper off once the danger passes.
The "taper off" part matters enormously. A helpful image comes from Robert Sapolsky's Why Zebras Don't Get Ulcers: a zebra fleeing a lion floods with stress hormones and then, once safe, returns to grazing, while humans secrete those same hormones over a thirty-year mortgage. When the stressor never lets up, the body's resources genuinely run down. Persistently elevated cortisol has been linked to disrupted sleep, weight changes, weakened immune function, and difficulty concentrating (see the American Psychological Association's overview of the stress response and chronic stress).
This is why chronic anxiety can leave you feeling physically depleted. You're running your emergency system as though it were your everyday one.
The Vagus Nerve: Your Built-In Brake
You can't think your way calm as fast as you can breathe your way there.
If the sympathetic nervous system is the gas pedal, your parasympathetic nervous system is the brake. And its most important cable is the vagus nerve, a long, wandering nerve that connects your brainstem to your heart, lungs, and gut.
Polyvagal theory places the vagus nerve at the center of whether we feel safe or threatened (polyvagal theory review, 2018). When the vagus nerve is engaged, your heart rate slows, your breathing deepens, and your body shifts into "rest and digest." When it's suppressed by chronic stress, that calming brake is harder to find.
The good news is practical: the vagus nerve responds to things you can do. Research on the physiology of slow breathing shows that a long, unhurried exhale shifts the autonomic nervous system toward its calming branch and quiets the stress response (slow breathing physiology, 2006). This is the concrete, measurable reason a slow breath out really does help.
Why Your Muscles Ache and Your Jaw Hurts
Anxiety braces the body. When your nervous system expects a threat, your muscles tense to prepare for action, shoulders creeping toward your ears, jaw clenching, fists tightening, stomach hardening.
Held over weeks and months, this bracing shows up as very real physical symptoms:
Tension headaches and a tight, sore neck
Jaw pain or teeth grinding, often at night
Back and shoulder pain
A jittery, restless feeling that won't settle
Fatigue from muscles that never fully relax
Many people go to their doctor for these symptoms without ever connecting them to anxiety. The body was keeping the tally quietly all along.
The Gut: Your Second Brain
Ever notice that anxiety lives in your stomach? That's not a metaphor. Your gut is lined with a dense network of neurons, the enteric nervous system, that communicates constantly with your brain along the gut-brain axis.
When you're anxious, blood flow to digestion drops and the gut becomes more reactive, which is why anxiety so often shows up as nausea, cramping, appetite changes, or urgent trips to the bathroom. The National Institute of Mental Health notes that physical symptoms like these are among the most common features of anxiety disorders. Neuroscientist Emeran Mayer, reviewing the research, described just how tightly the two systems are woven together, the gut sending far more signals up to the brain than the brain sends down, shaping mood and emotion in ways we're only beginning to map (Mayer, 2011, Nature Reviews Neuroscience).
Understanding this can be a relief. The churning in your stomach is your alarm system rerouting resources.
When the Body's Signals Get Misread
Here's where anxiety gets sneaky. Because these physical sensations feel so intense, your brain often reads them as evidence that something is wrong, which triggers more alarm, which produces more sensations. A racing heart gets interpreted as "I'm having a heart attack," which spikes adrenaline, which speeds the heart further.
This feedback loop is at the heart of panic. Learning that a pounding heart is uncomfortable but not dangerous, that the sensation will crest and pass on its own, is often the first step in breaking the cycle. That is the body's half of a loop the mind runs too, where the sensation becomes evidence and the worst case arrives fully formed a second later.
How to Calm an Anxious Body: A Self-Help Guide
You can't argue your nervous system out of an alarm state, but you can speak to it in its own language, the language of the body.
Lengthen your exhale. Breathe in for four counts, out for six or eight. A longer exhale directly engages the vagus nerve and signals safety, the same physiology the slow-breathing research describes.
Ground through your senses. Name five things you see, four you hear, three you can touch. This pulls the brain out of future-focused worry and into the present moment.
Move the adrenaline through. Walking, stretching, or shaking out your limbs helps metabolize the stress chemistry your body has already released, completing the response your body prepared for.
Use temperature. A warm shower or cool water on your face can nudge the nervous system toward calm, especially during a spike.
Name it plainly. Saying "this is anxiety; my body is protecting me" reduces the fear-of-the-fear that fuels the panic loop.
Protect the basics. Steady sleep, regular meals, and limits on caffeine give your nervous system fewer reasons to keep the alarm primed.

Treating the Body Along With the Thoughts
The sensations of anxiety are loud, but loud is not the same as dangerous.
The self-help tools above are useful, and I hope you use them. But they tend to work on the current alarm. When your body has been sounding that alarm for years, there's usually an older reason underneath. And that's where therapy offers something a breathing exercise can't reach on its own. Working on the older reason instead is most of what anxiety therapy consists of, week to week.
Chronic anxiety is often the nervous system remembering old danger rather than reacting to a present one. In my relational, psychodynamic, and attachment-based work, we get curious about why your body learned to stay so ready. What it was protecting you from, and what it never got to finish processing. Naming and working through those origins, inside a safe relationship, does something self-help can't: it lets the alarm finally update its information.
Therapy also offers co-regulation, the felt experience of another calm nervous system alongside yours, teaching your body over many sessions that it's safe to stand down. In my practice, I often help clients get curious about their physical symptoms instead of afraid of them, and that shift alone can change everything. Your body has been working hard to keep you safe. With the right support, it can learn to rest.
